The form organizations and processed was re-organized for the 2018 band season. The goal was to simplify and reduce the amount of information to record, and to give everyone a better sense of which exactly forms are required. (Some forms are information only).
To also reduce paper handling, we are requiring that most of the forms be turned in as a complete packet. Exceptions are noted below. Previously, forms were submitted individually which created a lot of extra work. If a needed form is missing or lacks a signature, etc., we'll return the entire packet so that the deficiency to be corrected.
Please note that it is a Pinellas school system requirement that all forms be submitted every year even if no information has changed.

Please note that some of the forms have special signature requirements such as a notary or a physician. Failure to have the correct signatures is a common deficiency problem. Due Dates for 2018
All forms by the start of band camp. It is a Pinellas school system requirement that all forms are submitted before a student is allowed to participate in the extracurricular activity. Please let a board member know if you expect to have any delays.
Band fee payment dates are noted in the materials.

Included in the collection is 02 Band Member Information Form. If you are a late joiner to the band, please turn this form in ASAP! It helps us to get your name and contact information in our systems more quickly.
Band Boosters – Check Request / expense reimbursement form. For anyone who pays out of pocket for a band item and needs to request a check reimbursement, PLEASE make sure you use the check reimbursement form and staple the original receipt to the form before you submit it to our treasurer.
BAND CAMP MENU CHOICES 2018 (tbd - The menu hasn't been prepared when it is ready, it will be available for downloading. Week(s) of (tbd) The Band Boosters will arrange for different restaurants to provide brown bag lunches during camp days. You will need to order and pay in in advance. You do not need to purchase lunch every day.
